Dear parent/carer,
Your daughter/son in year 8 was due to receive the first dose of the two dose Human Papilloma Virus (HPV) immunisation schedule earlier this year in school. Year 9 girls are also due their second and final dose of HPV. Welsh Government have identified Immunisation as a key priority to be continued, to prevent further infectious diseases. We have identified Willows High School, following a detailed risk assessment as a safe venue to deliver the immunisation.
Please return a completed consent form ASAP and return it to school.

1.00 – 1.30 pm: 8 Sutherland and Willows 2
1.30 – 2.15 pm: 8 Novello
Year 9 girls
Times:
2.15 – 2.45 pm: 9 Banksy and 9 Innes
2.45 – 3.15 pm: 9 Dali , Willows 2 Shakespere, 9 Hepworth and 9 Sherman
3.15 – 3.45 pm: Willows 2 Bronte and 9 Matisse
Pupils to arrive via the pupil entrance on Mercia Road and make their way to the canteen. They will exit via the car park on Willows Avenue.
